Job summary

Road To Responsibility, Inc. is hiring a licensed practical nurse (LPN) to provide general nursing care in residential services in Massachusetts. A $5,000 sign-on bonus is offered for new full-time hires.

You will administer medications, monitor health status, document progress, and coordinate care with DDS and families. This role requires an active LPN license, at least one year of related experience, valid driver's license, and willingness to complete mandated trainings.

Qualifications

• Current LPN License.
• Minimum of one year related experience.
• Valid Driver’s License.
• Basic knowledge of Microsoft Word and the internet.
• Successful completion of mandated trainings including medication administration rules and regulations.
• Willingness to authorize a Criminal History Background Check (CORI) and release to DPPC/DDS and other agencies.
• Must be able to perform essential duties; reasonable accommodations may be provided.

Responsibilities

• Explains procedures and treatments to individuals to gain cooperation and understanding.
• Administers prescribed medications and treatments per MAP guidelines.
• Maintains awareness of comfort and safety needs of individuals.
• Observes individuals, records significant conditions and notifies supervisor or physician.
• Takes vital signs to detect deviations and assess conditions.
• Responds to life-saving situations per nursing standards and protocols.
• Oversee medication administration within the program according to DDS regulations.
• Liaises with DDS, Day Program, and families around medical care.
• Complete/update medical assessments annually as needed.
• Prepare monthly progress notes for each participant.
• Assist with personal care tasks (diet, exercise, meals, bathing, dressing, etc.).
• Assist with physical direct care (transfers, mobility, etc.).
• Conduct on-site medical-based trainings for staff.
• Participate in on-the-job training and coaching of new staff.
• Adhere to RTR policies and procedures.
