Northeast Arc in Topsfield, MA is seeking a Licensed Practical Nurse (LPN) to join our residential care team. You will provide direct patient care, follow the plan of care, monitor health status, administer medications, and collaborate with the RN Case Manager and residential staff to support individuals in their community residence.

The role is full-time (40 hours per week) with an hourly pay of $42.00 and a $3,000 sign-on bonus paid within 12 months.

Qualifications

• Licensed LPN in Massachusetts with minimum of 1 year of experience.
• Experience working with individuals with intellectual or developmental disabilities preferred.
• Ability to develop and maintain respectful, caring, tolerant, ethical and empowering relationships with individuals with intellectual and physical disabilities.
• CPR certification and required trainings must be maintained.
• Must have a valid driver’s license and reliable transportation.

Responsabilités

• Health Monitoring & Care: conducts regular health assessments, vital signs, and reports to RN or on-call RN.
• Medication Management: administers medications and treatments per MAP regulations; manages documentation.
• Trach care, vent care, and related emergency procedures.
• Care Plan Implementation: completes written records, daily data collection, incident reports, progress notes.
• Medical Coordination & Communication: assists with medical appointments and ensures orders are followed.
• Change of shift reports for incoming staff.
• Compliance: maintains universal precautions and required certifications.
