Job summary

Road to Responsibility in Hanover, MA seeks a licensed practical nurse (LPN) for a full-time role. You will administer medications, monitor patient conditions, and coordinate with DDS and families to ensure safe care.

The position includes a $5,000 sign-on bonus and requires a current LPN license and one year of related experience. The job requires driving, excellent English communication, and compliance with state regulations.

Qualifications

• Current LPN license is required.
• Minimum of one year related experience.
• Valid Driver's License.
• Basic knowledge of Microsoft Word and the Internet.
• Completion of mandated trainings including medication administration rules.
• Willingness to authorize a Criminal History Background Check (CORI) and release of information concerning DDS/DPPC.
• Must be able to perform duties with reasonable accommodations.

Responsibilities

• Explains procedures and treatments to individuals to gain cooperation.
• Administers prescribed medications and treatments per guidelines.
• Monitors comfort and safety needs of individuals.
• Records vital signs and notifies supervisor of changes.
• Oversees medication administration within the program per DDS regulations.
• Liaises with DDS, Day Program, and families for medical care.
• Completes annual medical assessments and monthly progress notes.
• Assists with personal care tasks and transfers as needed.
• Provides on-site medical training to residential staff.
• Adheres to RTR policies and procedures.
