Heritage Inn in Statesboro, GA is seeking a part-time LPN RAI Coordinator for day shifts. The role focuses on coordinating care plans, physician and family communication, and ensuring compliant patient assessments. Requires valid Georgia LPN license and nursing degree or equivalent.

Qualifications

• Nursing degree from an accredited program or completion of an approved LPN program.
• Active LPN license to practice in Georgia.
• Proficient with Microsoft Office and basic computer skills.

Responsabilités

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan.
• Maintains schedule and coordinates care plan meetings with staff, family, and therapists.
• Assists Director with interviews, evaluations, and development of patient care plans.
• Ensures care plans align with regulations and are documented with timely signatures.
