Job summary

Heritage Inn in Statesboro, GA is seeking a Part-Time Day LPN to join our mission-driven team. You will facilitate interdisciplinary communication, assist with patient assessments, and help develop and implement comprehensive care plans in alignment with patient wishes and regulatory standards.

The role offers a starting pay of $30.49 to $33.40 per hour, with shift differentials for evenings, nights and weekends, and weekly pay.

Qualifications

• Nursing degree or LPN program completed.
• Active LPN license to practice in Georgia.
• Strong communication and organizational abilities.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to support an effective assessment/care plan process.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient assessments, care plans and evaluations.
• Ensures care plans align with patient wishes and regulatory requirements.
• Coordinates education for staff on care plans and MDS processes.
