Qualifications

• Nursing degree or LPN program graduate.
• Active LPN license to practice in Georgia.
• Proficient with Microsoft Office suite (Outlook, Excel, Word).
• Excellent organizational and prioritizing skills.
• Effective verbal and written communication.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to provide an effective assessment/care plan.
• Assists Director with patient interviews for assessment and with evaluation of each patient’s condition.
• Develops and coordinates care plans with measurable goals.
• Monitors timeliness and accuracy of assessments and care plans per regulations.
• Educates interdisciplinary team on MDS processes as needed.
