Job summary

Four County Health & Rehab is seeking a part-time nursing professional who will support the assessment/care plan process and coordinate with interdisciplinary teams to ensure high-quality patient care. The role emphasizes communication, timely care planning, and adherence to regulatory standards.

The position offers evening, night, and weekend differentials, weekly pay, and a range of benefits including PTO, holidays, vision, 401(k), and a referral bonus program.

Qualifications

• Must have a Nursing Degree from an accredited college or university or be a graduate of an approved LPN program.
• Active LPN License to practice in the State of Georgia is required.
• Excellent communication, organization and interpersonal skills are essential.

Responsibilities

• Facilitates communication between nursing center departments and patient/family members to provide an effective assessment/care plan process.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient or family interviews for assessment purposes and with development of care plans.
• Coordinates the review of each assessment/care plan for accuracy and timeliness.
• Monitors the MDS process for accuracy according to MDS guidelines and standards.
• Promotes the image and reputation of the System through servant leadership and open communication.
