Responsibilities

• Facilitates communication between nursing center departments and patient/family to support care planning.
• Establishes open lines of communication with consultants and interdisciplinary team members.
• Assists Director with patient interviews, assessment, and care plan development.
• Ensures care plans are accurate, timely, and signed/dated.
• Supports education for staff on patient care plans and MDS processes.
