Job summary

TMG is seeking a Licensed Practical Nurse (LPN) to support the Quality Data & Patient Outreach team. The role involves entering AWV-related orders, making patient outreach calls, and coordinating with practices to schedule patients, ensuring care gaps are addressed promptly.

The position requires an Associate degree or 2 years of college coursework, and an active LPN license. Outpatient, data, and EMR experience are preferred to support providers effectively.

Qualifications

• Formal education program in practical nursing completed.
• Current state LPN licensure.
• Outpatient, quality, or population health experience preferred.
• Experience with EMRs, data abstraction, patient outreach, or value-based care initiatives preferred.

Responsibilities

• Enter and prepare AWV-related orders in advance of patient visits.
• Conduct outreach calls to patients.
• Review documentation for completeness for providers.
• Coordinate with practices for scheduling patients.
• Collaborate with providers, managers, and directors to address care gaps in a timely manner.
