Tanner Health is seeking an LPN to support our Quality and Data Analytics team. The role involves entering AWV-related orders ahead of visits, conducting outreach to patients, and reviewing documentation for provider completeness.

• Education: Associate Degree or 2 years of college coursework.
• Experience: One year of related experience.
• Licensure: Alabama Licensed Practical Nurse (LPN) or multistate license.
• Prior outpatient, quality, or population health experience strongly preferred.
• Experience with EMR, data abstraction, patient outreach, or value-based care initiatives preferred.

Responsabilidades

• Enter and prepare AWV-related orders in advance of patient visits.
• Conduct outreach calls to patients.
• Review documentation for completeness for providers.
