Job summary

Tanner-Health-1 is seeking an LPN to support our Quality and Data Analytics team. Primary duties include entering AWV-related orders, conducting outreach calls to patients, and reviewing documentation for providers.

You will coordinate with practices to schedule patients and work with TMG providers, managers, and directors to address care gaps in a timely manner. Strong attention to detail and EMR experience preferred; outpatient, quality, or population health experience is a plus.

Qualifications

• Completion of a formal education program in practical nursing.
• Current state LPN licensure.
• One year of related experience preferred.
• Experience with EMR's, data abstraction, patient outreach, or value-based care initiatives preferred.
