Job summary

TMG is seeking a Licensed Practical Nurse (LPN) to support the Quality Data & Patient Outreach team. The role involves entering AWV-related orders, making patient outreach calls, and coordinating with practices to schedule patients, ensuring care gaps are addressed promptly.

The position requires an Associate degree or 2 years of college coursework, and an active LPN license. Outpatient, data, and EMR experience are preferred to support providers effectively.

Job Description

This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include entering and preparing AWV-related orders in advance of patient visits, conducting outreach calls to patients, reviewing documentation for completeness for providers, and coordinating with practices for scheduling patients. This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.

Required Knowledge & Skills

Education: Associate Degree or 2 years of college coursework

Experience: One year of related experience. Requires a working knowledge of standard practices and procedures.

Licenses and Certifications

• ALABAMALICENSED PRACTICAL NURSE OR MULTISTATE NURSING LICENSE

Qualifications

• Completion of a formal education program in practical nursing.
• Current state LPN licensure.
• Prior outpatient, quality, or population health experience strongly preferred.
• Experience with EMR's, data abstraction, patient outreach, or value-based care initiatives preferred.
