Tanner Health is seeking an LPN to support our Quality and Data Analytics team. The role involves entering AWV-related orders ahead of visits, conducting outreach to patients, and reviewing documentation for provider completeness.

You will coordinate scheduling with practices and work with TMG providers, managers, and directors to address care gaps promptly. The ideal candidate has an LPN licensure in Alabama (or a multistate license), an Associate Degree or equivalent, and at least one year of

• Experience: One year of related experience.
• Licensure: Alabama Licensed Practical Nurse (LPN) or multistate license.
• Prior outpatient, quality, or population health experience strongly preferred.
• Experience with EMR, data abstraction, patient outreach, or value-based care initiatives preferred.

This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include entering and preparing AWV-related orders in advance of patient visits, conducting outreach calls to patients, reviewing documentation for completeness for providers, and coordinating with practices for scheduling patients. This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.
Required Knowledge & Skills
Education: Associate Degree or 2 years of college coursework
Experience: One year of related experience. Requires a working knowledge of standard practices and procedures.
Licenses and Certifications

• ALABAMA LICENSED PRACTICAL NURSE OR MULTISTATE NURSING LICENSE

• Completion of a formal education program in practical nursing.
• Current state LPN licensure.
