Job summary

Tanner Health is seeking an LPN to support our Quality and Data Analytics team. You will enter and prepare AWV-related orders ahead of visits, reach out to patients, and review documentation for provider completeness.

You will coordinate with practices for scheduling patients and work with TMG providers, managers, and directors to address patient care gaps promptly.

Qualifications

• One year of related experience.
• Education: Associate Degree or 2 years of college coursework
• Requires a working knowledge of standard practices and procedures.
• Formal education in practical nursing completed.
• Current state LPN licensure.
• Outpatient, quality, or population health experience strongly preferred.
• Experience with EMR, data abstraction, patient outreach, or value-based care initiatives preferred.

Job description

This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include entering and preparing AWV-related orders in advance of patient visits, conducting outreach calls to patients, reviewing documentation for completeness for providers, and coordinating with practices for scheduling patients. This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.
Required Knowledge & Skills

Experience: One year of related experience. Requires a working knowledge of standard practices and procedures.
Licenses and Certifications

• ALABAMA LICENSED PRACTICAL NURSE OR MULTISTATE NURSING LICENSE
