Qualifications

• Licensed Practical Nurse (LPN) required.
• Maintains BLS certification (AHA).

• Experience coordinating patient flow or clinical operations preferred.
• Strong communication and interpersonal skills.
• Strong organizational skills to manage multiple priorities.
• Proficiency with EHR and Microsoft Office.

Responsibilities

• Oversee daily patient flow to support timely care.
• Serve as liaison between patients, physicians, and staff.
• Facilitate communication on patient needs and scheduling.
• Support physicians and staff with daily program activities.
• Assist the program manager with operations and scheduling.
• Monitor program activities to identify issues.
• Coordinate with internal and external providers for continuity.
• Maintain accurate patient and program documentation.
• Provide patients and families with information about services.
• Resolve patient concerns and escalate issues appropriately.
• Collaborate with the clinical team to address needs promptly.
• Support policy implementation and process improvements.
• Maintain HIPAA compliance.
