Job summary

You will liaise with patients, physicians, and clinical staff, coordinate scheduling and program activities, maintain accurate documentation, and support quality initiatives while upholding HIPAA and regulatory requirements.

Qualifications

• Bachelor's degree in healthcare-related field preferred.
• Licensed Practical Nurse (LPN) required.
• Maintains American Heart Association Basic Life Support.
• Clinical knowledge of patient care processes.
• Healthcare or patient-facing experience required.
• Experience coordinating patient flow or clinical operations preferred.
• Proficient with EHRs and Microsoft Office.
• Strong communication, organization, and judgment.

Responsibilities

• Oversee daily patient flow and coordinate movement through the program to support timely and efficient care.
• Serve as a primary liaison between patients, physicians, clinical staff, and the program manager.
• Facilitate communication regarding patient needs, appointments, scheduling, and program-related concerns.
• Support physicians and clinical staff with coordination of daily program activities and patient needs.
• Assist the program manager with day-to-day operations, workflow, scheduling, and administrative functions.
• Monitor program activities and identify operational issues that may affect patient flow or the patient experience.
• Coordinate with internal departments and external providers to ensure continuity and efficiency of services.
• Maintain accurate patient and program documentation in accordance with organizational policies and regulatory requirements.
• Provide patients and families with information regarding program services, processes, and resources.
• Assist with resolving patient concerns and escalating clinical or operational issues appropriately.
• Collaborate with the clinical team to ensure patient needs are communicated and addressed in a timely manner.
• Support the implementation of program policies, procedures, quality initiatives, and process improvements.
• Maintain confidentiality and comply with HIPAA, organizational policies, and applicable healthcare regulations.
