Job summary

JourneyCare Hospice in Lake Bluff, IL is seeking a PRN Hospice LPN to join the Crisis Care team. New competitive salaries and flexible hours are offered, with shifts up to 12 hours (7PM–7AM). Compensation is up to $38 per hour, depending on experience.

Responsibilities include direct patient care under the Nurse Practice Act, documentation, patient/family education, and coordination of hospice services with the interdisciplinary team. A valid IL LPN/LVN license and CPR certification are required.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Current LPN/LVN license in Illinois.
• CPR certification required.
• Demonstrates strong verbal and written communication and organizational skills.
• Valid driver’s license with insured vehicle.

Responsibilities

• Provide direct patient care as defined by State Nurse Practice Act.
• Implement current nursing practice following an assessment and plan of care.
• Document patient services timely to reflect the plan of care.
• Educate patients and families/caregivers about diagnosis and self-plan of care.
• Coordinate hospice services, reporting needs to the interdisciplinary group.
• Manage pain and symptoms and monitor treatment responses.

HOURS: As Needed. Shifts will be 12-hour shifts 7PM-7AM. Possibility of both weekdays and weekends.

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill
