Job summary

JourneyCare Hospice in Libertyville, IL seeks a Hospice LPN to join the Crisis Care team on a PRN basis. Provide direct patient care under the State Nurse Practice Act and assist with care planning and documentation.

Shifts are 12 hours, 7PM-7AM, with potential weekdays and weekends. Competitive pay up to $38/hour based on experience; flexible hours, mileage reimbursement, and a supportive team culture are provided.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience, hospice/community health preferred.
• Current LPN/LVN license in the practicing state; CPR certification.
• Excellent verbal and written communication and organization skills.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act.
• Implement the plan of care based on comprehensive assessment.
• Document patient services accurately and timely.
• Educate patient and family/caregiver on diagnosis and self-care.
• Coordinate hospice services and report needs to the interdisciplinary group.
• Manage pain and symptoms and evaluate treatment responses.

As Needed. Shifts will be 12-hour shifts 7PM-7AM. Possibility of both weekdays and weekends.

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill

What You’ll Do:

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
