Job summary

JourneyCare Hospice is seeking a Hospice LPN to join its Crisis Care team in Crystal Lake, Illinois. This role offers competitive salaries up to $38 an hour and flexible hours, with shifts being 12 hours from 7 PM to 7 AM.

The ideal candidate will have at least three years of nursing experience, preferably within community health or hospice, and must be currently licensed as an LPN in Illinois.

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program.
• Three years of nursing experience, preferably in community health or hospice.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Valid driver’s license with insured automobile.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Document patient services accurately and timely.
• Educate patients and families pertinent to diagnosis.
• Coordinate hospice services with the interdisciplinary group.
• Manage pain/symptoms and evaluate patient responses.
