Job summary

JourneyCare Hospice in Lake Forest, IL is seeking a Hospice LPN to join our growing team. This field-based role follows the plan of care, provides treatments, and collaborates to support patients and families.

Hours are Monday–Friday 8:30–5:00 with no on-call or weekends. Salary ranges from $26 to $38 per hour with offer based on years of experience. This position includes benefits such as medical/dental/vision, 401(k) with match, PTO, mileage and more.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical preferred.
• Current LPN/LVN license in the practicing state.
• CPR certification is required.
• Valid driver’s license with an insured vehicle.
• Demonstrates good verbal and written communication and organization skills.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services reflecting the plan of care.
• Assess and provide patient and family education pertinent to diagnosis and self-plan of care.
• Coordinate hospice services and report identified needs to the interdisciplinary team.
• Provide pain and symptom management and evaluate responses to treatments.

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program

• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill

What You’ll Do:

• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
