Job summary

JourneyCare Hospice in Lake Forest, IL is seeking a Hospice LPN to join our growing team. This field-based role follows the plan of care for our patients, provides treatments, and collaborates with families.

Hours are Monday–Friday 8:30-5pm with no on-call or weekends. Salary is $26-$38 hourly, based on experience. We offer comprehensive benefits including medical coverage, 401(k), PTO, mileage reimbursement, tuition assistance, referral bonuses, and career growth opportunities.

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program.
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Demonstrates good verbal and written communication, and organization skills.
• Valid driver’s license with an automobile that is insured in accordance with state requirements.
• Understands the philosophy of hospice concept and the needs of the terminally ill.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications.
