JourneyCare Hospice is seeking a Hospice LPN to join our Crisis Care team on a PRN basis with competitive salaries and flexible hours. Now offering up to $38 an hour, based on experience.

The LPN/LVN will provide direct patient care, follow the plan of care, document services, and educate patients and families. Requires Illinois LPN license, CPR certification, and reliable transportation.

Qualifications

• Graduate of an accredited practical nurse or a vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the practicing state; CPR certification required.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications.

As Needed. Shifts will be 12-hour shifts 7PM-7AM. Possibility of both weekdays and weekends.

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill
