Job summary

JourneyCare Hospice is seeking a Hospice LPN to join our Crisis Care team. This field-based role follows the plan of care for continuous care patients, delivering treatments and collaborating with families.

The LPN will work Monday–Friday with (3) 12-hour shifts 7 PM–7 AM and every other weekend, serving Northbrook, IL. We offer comprehensive benefits, a 401(k) match, PTO and tuition reimbursement.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess CPR Certification.
• Demonstrates good verbal and written communication, and organization skills.
• Valid driver’s license with a vehicle insured per state requirements.
• Understands the philosophy of hospice care and needs of the terminally ill.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Follow a comprehensive assessment and the plan of care.
• Document patient services accurately and promptly to reflect the plan of care.
• Educate patient and family/caregiver about diagnosis and self-care plans.
• Coordinate hospice services and report needs to the interdisciplinary team.
• Provide pain and symptom management and evaluate treatment responses.

JourneyCare Hospice is seeking a Hospice LPN to join our growing Crisis Care team! This is a field-based position responsible for following the plan of care for our continuous care patients, providing treatments, and working collaboratively to help our patients and their families.

Salary: $26 - $38 hourly - Offer Based On Years of Experience

Qualifications:

What You’ll Do:

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
