Job summary

Plan A Health is seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and adult patients in North Carolina. The role emphasizes applying the nursing process to deliver skilled care in the patient's home.

The LPN will assess, plan, implement, and evaluate care; administer nursing services per orders; monitor changes; and collect and document patient data. Active NC licensure and BLS/ TB clearance are required, with 1 year of experience preferred.

Qualifications

• Active LPN/LVN license in North Carolina.
• TB test or Chest X-ray and BLS certification.
• One year of professional nursing experience preferred.
