Job summary

Preferred Home Health Care & Nursing Services is seeking a licensed practical nurse (LPN) to provide in-home care for pediatric and/or adult patients in Irving, Texas. You will assess, implement, and evaluate care using the nursing process, delivering skilled nursing services per physician orders.

The role emphasizes patient-centered care, accurate documentation, and collaboration with a healthcare team. A licensed LPN in Texas with BLS and TB/CX-ray clearance is required; one year of experience

Qualifications

• Current LPN/LVN license in the practicing state.
• TB or Chest X-Ray and BLS certification.
• Preferred: at least one year of professional nursing experience.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Provide skilled nursing services based on prescribed orders.
• Monitor signs and symptoms to identify physiological and psychosocial changes.
• Collect and analyze data from the healthcare team and document nursing diagnoses.
