Job summary

Preferred Home Health Care & Nursing Services is seeking a Licensed Practical Nurse (LPN) to provide in-home care for pediatric and/or adult patients. You will assess, plan, implement, and evaluate care, delivering skilled nursing services according to orders in a home setting.

The role requires an active LPN/LVN license in the state, TB or Chest X-Ray and BLS certification, and ideally one year of nursing experience.

Qualifications

• Licensed LPN/LVN in the practicing state.
• Current TB or Chest X-Ray and BLS certification.
• Preferably, one year of professional nursing experience.

Responsibilities

• Assess, plan, implement, and evaluate patient care using the nursing process.
• Deliver skilled nursing services per prescribed orders.
• Monitor signs and symptoms to identify physiological and psychosocial changes.
• Collect and interpret data, documenting nursing diagnoses.
