Job summary

Humana Inc. is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate care among patients, field staff, and physicians. The role also supports day-to-day office and staff management and may participate in on-call rotations.

The position requires an active RN/LVN license, at least one year of home health experience, strong communication and organizational skills, and a valid driver’s license. Expect a 40-hour workweek with a competitive benefits package.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state where you practice.
• At least 1 year of home health experience.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the state of employment if applicable.
• Must possess excellent communication skills and strong organizational abilities, with the capacity to manage multiple assignments.

Responsibilities

• Manages schedules for all patients and edits schedules when agents are unavailable.
• Initiates infection control forms and forwards reports to the HRD.
• Prepares weekly agent schedules and reviews productivity/geographic issues.
• Assists with internal transfer of patients between branch offices and back-up during peak times.
• Reschedules missed or declined visits and ensures supervisory visits are scheduled.
