Job summary

Humana Inc. is seeking a Patient Care Coordinator to develop and maintain relationships with hospitals, SNFs, and discharge planning services, assisting in determining patient eligibility and coordinating resources.

You will conduct facility visits at the physicians' request to support the clinical team and participate in weekly business development meetings. The role requires an RN/LPN/LVN license, a BSN preferred, and about 3 years of clinical experience.

Qualifications

• RN/LPN/LVN license required.
• BSN preferred.
• 3 years clinical experience preferred.
• Strong knowledge of governmental regulations and Medicare eligibility requirements.
• Excellent customer service, communication and selling skills.
• Willingness to travel within assigned territory and to sales meetings.
• TB screening program participation if selected.

Responsibilities

• Develop and maintain contact with hospital, skilled nursing and discharge planning services.
• Conduct facility visits at physician requests to support program clinical team.
• Coordinate with program staff and field sales resources in weekly meetings.
• Assist program in processing physician orders as directed.
• Report to a Market Executive of Sales and participate in business development meetings.
