Job summary

Humana / CenterWell Home Health seeks a licensed LPN/RN to coordinate patient visits and ensure continuity of care. You will schedule staff, liaise with field staff and physicians, and assist with daily office operations to support patient outcomes.

The ideal candidate has 1+ year of home health experience, excellent communication and organizational skills, a valid driver's license, and the ability to multitask in a dynamic home health setting.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state of practice.
• Have at least 1 year of home health experience.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Must possess excellent communication skills, the ability to interact well with a diverse group, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Schedules visits and coordinates with field staff, patients and physicians to maintain care continuity.
• Maintains and edits schedules; handles reassignment when staff are unavailable.
• Initiates infection control forms and handles HRD reporting as needed.
• Assists with office and staff management and participates in weekly case conferences.
• Runs scheduling reports and prepares weekly schedules for the Branch Director.
• Supports transfer of patients between branch offices and on-call activities if clinical.
