This role links clinical staff with patients and families, coordinating scheduling and treatment plans, explaining medical information in clear terms, and supporting on‑going care across the care team. You will manage patient schedules, coordinate visits and transfers, assist with case conferences, and communicate with field staff.

Qualifications

• Active RN/LPN license in the state where you practice.
• Minimum 1 year of Home Health experience preferred.
• Coding certification is a plus.

Responsibilities

• Schedule and manage patient appointment and visit calendars.
• Coordinate discharge visits, OASIS collection, recert visits, and transfers to inpatient facilities.
• Reschedule declined or missed visits and reassign/re-schedule visits.
• Prepare and edit weekly agent schedules and review productivity/geographic issues.
• Assist with weekly case conferences and field communications.
