Job summary

McLaren Health is seeking a Licensed Practical Nurse (LPN) to work under the direction of the Team Leader. The roleGatheres assessment data, monitors patient needs, follows the plan of care, and administers medications.

You will supervise CENAs, collaborate with care coordinators, and communicate changes in residents' conditions in a timely manner to support high-quality nursing care. Ideal candidates will hold Michigan LPN licensure and have graduated from an accredited nursing program.

Qualifications

• Graduate of an accredited school of nursing.
• License to practice practical nursing in the State of Michigan OR completion of a U.S. Armed Forces Practical Nursing Program.

Responsibilities

• Gathers assessment data, monitors comprehensive physiological, psychological, sociocultural, and spiritual needs of patients/significant others under the supervision of the Team Leader. Reports findings to the Team Leader.
• Provides competent care consistent with the nursing process and written plan of care, physician's orders and in accordance with skilled nursing facility policies and procedures.
• Participates in admission and discharge planning, in collaboration with health care team, at the time of admission and continues through discharge.
• Provides patient and family education to meet their specific needs as outlined in the plan of care and delegated by the Team Leader.
• Documents care consistent with guidelines.
