Job summary

McLaren Health is seeking an experienced Licensed Practical Nurse (LPN) to join our skilled nursing team in Michigan. The LPN will support the Team Leader by gathering assessments, monitoring resident conditions, and delivering care per the care plan.

You will administer medications, supervise CENAs, educate residents and families, document care, and collaborate with care coordinators to ensure timely communication and updates on changes in condition.

Qualifications

• Graduate of an accredited nursing program.
• Licensed to practice practical nursing in Michigan.
• Excellent communication and teamwork skills.

Responsibilities

• Gathers assessment data and monitors patients under the Team Leader.
• Delivers care using the nursing process and written plan of care.
• Administers medications and treatments per physician orders.
• Supervises CENAs performing activities of daily living.
• Collaborates with care coordinators to update changes in condition.
• Educates patients and families per the care plan.
• Documents care in the medical record.
