Job summary

McLaren Health is seeking a Licensed Practical Nurse (LPN) to work under the direction of the Team Leader in Michigan. The LPN will gather assessment data, monitor residents, administer medications, and participate in care planning and education for patients and families.

Responsibilities include collaborating with the Care Coordinators and other team members, documenting care, and ensuring timely updates on changes in residents’ conditions in accordance with facility policies and procedures.

Qualifications

• Graduate of an accredited school of nursing.
• License to practice practical nursing in Michigan or completion of a U.S. Armed Forces Practical Nursing Program.

Responsibilities

• Gathers assessment data and monitors patients under supervision, reporting findings to the Team Leader.
• Provides nursing care per plan of care and physician orders in line with facility policies.
• Participates in admission and discharge planning with the health care team.
• Educates patients and families according to the plan of care.
• Documents care accurately and maintains regulatory compliance.
• Performs other duties as assigned.
