Job summary

Community Care, Inc. is seeking a compassionate LPN to provide one-on-one pediatric private duty nursing for an 11-year-old in Connellsville, PA.

This role focuses on individualized care, safety, and building trusting relationships with the family. Responsibilities include ADLs, g-tube care, monitoring vitals and seizures, oxygen administration, medication administration, and clear communication with the care team.

Qualifications

• Current LPN license in Pennsylvania.
• Graduate of an accredited nursing program.
• Minimum 6 months nursing experience; home health preferred.
• CPR certification required.
• Reliable transportation and a valid driver's license.

Responsibilities

• Assist with Activities of Daily Living (ADLs).
• Perform g-tube care and feedings.
• Monitor vitals and seizures.
• Administer oxygen as prescribed.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
