Job summary

Qualifications

• Must hold a current and valid LPN or RN license in New Hampshire.

Responsibilities

• Supervising and guiding direct care professional activities to ensure quality care
• Participating in admissions, discharges, and transfers
• Monitoring resident conditions and promptly notifying physicians and families
• Accurately transcribing and carrying out physician orders
• Administering medications and monitoring for adverse reactions
• Performing wound care and treatments per physician orders
• Coordinating follow‑up appointments and transportation
• Providing direct nursing procedures such as tube feedings, suctioning, IV administration, colostomy care, and more
• Conducting rounds to ensure care needs are met and staff duties are completed
• Collaborating with interdisciplinary team members to support comprehensive care
• Maintaining adequate supplies, medications, and equipment
• Initiating and updating care plans
• Supporting end‑of‑life care with compassion and professionalism
• Assisting during surveys and participating in committees as assigned

Compensation: LPN $31.00 - 38.00 | RN $43.00 - $50.00, based on experience

What You’ll Do
