Qualifications

• Current state LPN license and CPR certification.
• 1–2 years nursing experience preferred.
• Experience with elderly care is desirable.

Responsibilities

• Accurately assesses, communicates, and documents residents’ status.
• Observes and reports changes in resident health to care team per state regulations.
• Directs and supervises daily work of Resident Care Assistants.
• Mentors Lead RCAs in all aspects of their job.
• Coordinates care needs with community providers via case management.
• Balances service demands with resident independence.
• Reports elder abuse/neglect per state regulations.
• Responds to emergencies and uses the emergency system as needed.
