Signature HealthCARE in Chapel Hill, NC, is seeking a caring LPN/LVN to provide direct nursing care, manage medications, and support residents with ADLs. You will supervise nursing assistants, document care, and participate in care planning with the interdisciplinary team.

This role emphasizes resident-centered care, regulatory compliance, and quality outcomes in a facility focused on dignity and safety. Candidates with LTAC experience and supervisory background are encouraged to apply.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Knowledge of Federal and State long-term care regulations.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift per policy.
• Monitor residents’ status; notify physician and responsible party of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
