Job summary

Signature HealthCARE is seeking an experienced LPN/LVN to provide direct nursing care in a dynamic long-term care setting in North Carolina. You will supervise nursing assistants, manage medications, and participate in care planning to ensure high-quality outcomes.

The role requires active licensure, strong clinical skills, and the ability to work collaboratively within an interdisciplinary team. Prior supervisory experience and familiarity with EHR systems are a plus.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy.
• Coordinate resident admissions, transfers, and discharges per procedures.
• Participate in QA activities, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
