Monarch Healthcare Management is seeking a Licensed Practical Nurse to provide direct routine care to residents under nursing leadership and help tailor care plans. The LPN will collect health data, report changes, and support the Plan of Care implementation across Monarch's Minnesota facilities.

The role emphasizes teamwork with RNs and physicians, ensuring residents achieve the highest practicable physical and psychosocial well-being in a supportive work environment.

Qualifications

• Must have completed a nursing education program approved by the Minnesota Board of Nursing.
• Must hold a current Minnesota Practical Nurse license.
• Must maintain current CPR certification.

Responsibilities

• Provide residents with direct routine care under nursing leadership.
• Participate in development and modification of the Plan of Care.
• Collect and monitor resident health data and report changes to the RN.
• Assign and monitor nursing tasks to unlicensed staff as appropriate.

Education

Completion of a professional nursing education program approved by the Minnesota Board of Nursing

Current Minnesota License as a Practical Nurse

Current Certification in CPR
