Monarch Healthcare Management is seeking an LPN to provide high-quality direct nursing care to residents under the supervision of an RN. The role involves contributing to care plans, performing delegated interventions, and monitoring patient status in accordance with Minnesota BON standards.

Qualified candidates will hold an MN LPN license, CPR certification, and have experience in skilled nursing or assisted living facilities.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse (LPN).
• Current CPR certification for emergency care.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine nursing care under RN supervision.
• Assist in development and modification of the resident Plan of Care.
• Monitor resident health data and report changes to the RN.
