Job summary

Monarch Healthcare Management seeks an LPN to provide high-quality direct routine care to residents under RN supervision in accordance with Minnesota BON standards and the residents' rights. The LPN participates in care planning, collects health data, and communicates changes to the RN promptly.

Responsibilities include coordinating with licensed staff, guiding unlicensed aides, and contributing to the facility's QAPI-driven quality of care.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine nursing care under the supervision of the RN and in accordance with MN BON standards.
• Assist in developing and modifying the resident Plan of Care under RN guidance.
• Collect resident health data and report changes to the RN.
• Assign and monitor nursing tasks or activities of unlicensed staff as appropriate.
