Job summary

Monarch Healthcare Management is seeking a Licensed Practical Nurse (LPN) to provide direct routine care under RN supervision, participate in care planning, and monitor residents’ health status in a Minnesota facility.

The role emphasizes adherence to Minnesota BON standards, resident rights, and the facility's QAPI program, with opportunities for professional growth and a strong focus on resident well-being.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse (LPN).
• CPR certification and emergency care.
• Experience in skilled nursing and/or assisted living facilities.

Responsibilities

• Provide residents with direct routine care under the supervision of the RN.
• Develop and modify the resident Plan of Care with interventions delegated, ordered or prescribed by the RN or physician.
• Collect resident health data and report changes to the RN.
• Assign and monitor nursing tasks or activities of unlicensed staff.
• Support residents in achieving and maintaining the highest practicable physical, mental and psychosocial well-being.
