Job summary

Wellpath is seeking a Licensed Practical Nurse (LPN) to provide compassionate patient care in our Vermont facility. Under RN supervision, you will participate in assessment, planning, and documentation while supporting a warm, team-focused environment.

This full-time position requires current LPN/LVN licensure in the state, CPR certification, and a minimum of one year of clinic experience. Shift: Night 12 hour; compensation: $36.82–$45/hour; benefits and growth opportunities.

Qualifications

• Graduate from an accredited school of nursing.
• Prefer a minimum of one (1) year clinic experience.
• Maintain current LPN/LVN licensure in the state of employment; CPR certification required.

Responsibilities

• Under RN supervision, assesses patients' physical, psychological, and social dimensions, contributing to individualized treatment planning.
• Administers medications, including IV therapy (if certified), and performs diagnostic tests according to established protocols.
• Counts controlled substances at start of shift, adheres to safety techniques and standard precautions.
• Assists in procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting.
• Attends mandatory staff meetings and training to uphold high-quality patient care.
