Wellpath is seeking a Licensed Practical Nurse (LPN) in Vermont to contribute to direct patient care underRN supervision. The role focuses on assessment, medication administration, and adherence to care plans within a Nursing department.

Qualifications

• Graduate from an accredited nursing program.
• Current LPN/LVN licensure in the state of employment.
• CPR certification.
• This position is available only to United States residents.
• Prefer a minimum of one (1) year clinic experience.

Responsibilities

• Under RN supervision, assesses patients' physical, psychological, and social dimensions to aid treatment planning.
• Administers medications, including IV therapy if certified, per established protocols.
• Collaborates in controlled substance counts and follows safety practices and emergency protocols.
• Assists in medical or minor surgical procedures and documents nursing encounters.
• Attends mandatory staff meetings to support ongoing professional development and adherence to policies.

Education
