Job summary

Ascension is seeking an LPN for a day shift role in Wewahitchka, FL, at Ascension Sacred Heart Medical Clinic. You will work under RN or physician supervision to deliver direct patient care.

You will administer medications, monitor for adverse reactions, and assist with assessments while educating patients and families about discharge plans and self‑care. A sign‑on bonus is offered and full benefits apply.

Qualifications

• BLS Provider certification within 1 month of hire date.
• LPN license issued by the state where you will work.
• High School diploma or equivalent required; related licensure can substitute.
• Experience with patient care and routine nursing tasks is valued.

How you’ll make an impact in this role

• Provide care to patients under the direction of a registered nurse or physician, functioning within scope of license.
• Contribute to the assessment of patients and assure the well-being of patients.
• Administer medications and observe patients for adverse reactions to medications or treatments.
• Educate patients/families about disease treatment plan including self care post discharge, holistic health needs, available resources and follow up care.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• Required Credential(s):
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
  • One or more of the following: Licensed Practical/Vocational Nurse credentialed from the Alabama Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works.
  • Licensed Practical/Vocational Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works.
  • Licensed Practical/Vocational Nurse credentialed from the Georgia Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works.

Education:

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. *Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
