Job Overview

The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments safely and organized. Responsibilities include medication administration, documentation, conducting SHUs, and providing resident care, all in accordance with laws, standards, regulations, and Sunrise policies to ensure quality care for our residents.

Responsibilities & Qualifications

Responsibilities

• Support our Mission, Principles of Service, and Core Values, emphasizing the sacred value of human life.
• Administer medications and treatments, ensuring safety and proper documentation.
• Review, read, and notate the Daily Log for resident information.
• Receive medication updates from the RCD or Wellness Nurse.
• Administer, assist, and observe medications using the Six Rights of Medication Pass.
• Document medication administration, including refusals or missed doses.
• Maintain confidentiality of all resident information.
• Report concerns and assist with medication restocking and checking.
• Assess residents for 'as needed' medications and document appropriately.
• Count narcotics with another MCM or LCM each shift.
• Maintain cleanliness and organization of medication and treatment areas.
• Assist residents in maintaining independence and dignity.
• Practice good standard care precautions and routine safety procedures.
• Participate in team meetings and contribute to resident care planning.
• Ensure compliance with safety, OSHA, and infection control policies.
• Report accidents, hazards, and occupational exposures immediately.
• Participate in training and may serve as shift supervisor or supervise others.

Qualifications

• Licensed as LPN, LVN, or equivalent in the respective state/province.
• Maintain required certifications such as CPR and First Aid.
• Must be at least 18 years old.
• Experience with seniors preferred.
• Strong communication and judgment skills.
