Job Summary

The Medicare Advantage Respiratory Therapist Care Manager facilitates comprehensive respiratory care coordination for customers with chronic obstructive pulmonary disease (COPD) and/or asthma diagnoses. The role involves delivering quality healthcare services by identifying triggers, managing the customer's disease, and setting short- and long-term goals through advocacy, development, and implementation of individualized care plans.

Job Requirements:
  • Bachelor of Social Work or Psychology OR LVN, LPN with 1 year experience in managed care systems OR RN OR 3 years of care coordination for a state-managed or waiver program OR 3 years of managed care systems experience.
  • Current licensure as a Respiratory Therapist is required.
  • NBRC Registered Respiratory Therapist certification required.
  • Knowledge of medical terminology.
  • Experience coordinating member medical needs, providing assistance, and analyzing member needs.
  • Proficiency in PC applications, including Microsoft Office.
  • Strong customer service and customer-centric skills.
  • Excellent verbal and written communication skills, including developing correspondence to members and department personnel, and coaching skills such as motivational interviewing to educate members on medical issues.
  • Requires critical thinking, decision-making, analytical skills, and strong interpersonal relationship skills.
  • Knowledge of current healthcare practices and appropriate treatments.
  • Knowledge of community resources.

  • This is a telecommute (remote) role: Must reside within 250 miles of the listed offices or within the posted states.
