Qualifications

Job description

Join our mission to help transform healthcare delivery from reactive, episodic care to proactively managed patient care that prevents life-changing problems before they happen for patients with two or more chronic conditions. We believe every patient with chronic disease deserves consistent check-ins, follow-up, and support.

Position: Nurse Chronic Care Coordinator, Remote

This role involves telephonic encounters with patients on behalf of our partners each month and developing detailed care plans within our electronic health record templates. Initially, this is an Independent 1099 Contractor position, with potential to transition to full-time W2 employment with benefits.

Harris CCM seeks Nurses to work part-time from their home office, complying with HIPAA privacy laws. You will set your own hours, with a minimum commitment of 20 hours per week. This flexible model allows you to choose your work days and hours, balancing work and home life.

You will be assigned a patient panel based on your skills and efficiency, with a minimum of 100 patients per month, and expected to complete encounters on 90% of your assigned patients.

Compensation

We utilize a productivity-based pay structure:

  1. $10.00 per encounter up to 99 encounters/month
  2. $10.25 per encounter from 100-149 encounters/month
  3. $12.00 per encounter from 150-199 encounters/month
  4. $14.00 per encounter from 200-249 encounters/month
  5. $16.00 per encounter for over 250 encounters/month

Payment tier increases require three months of consistency. Each encounter takes a minimum of 20 minutes.

  • Enhance revenue through billable CCM episodes and increased management visits.
  • Develop detailed care plans for prevention and intervention.
  • Understand healthcare goals related to chronic disease management.
  • Attend scheduled meetings, including bi-monthly staff meetings and monthly one-on-ones.
Qualifications
  • Graduate from an accredited Nursing program (LPN, LVN, RN, BSN).
  • Current Oregon nursing license with no disciplinary actions.
  • At least two years of clinical experience in Med/Surg, Case Management, or home health care.
  • Experience with Electronic Medical Records and familiarity with Windows/Microsoft Office tools, working in HIPAA-compliant home environments.
  • Strong initiative, judgment, organization, and problem-solving skills.
  • Proficiency with computer programs; love for technology is essential.
  • High-speed internet and a Windows or Mac desktop/laptop (no Chromebooks).
  • Excellent verbal, written, and listening skills.
