Chronic Care Manager (Remote - Compact States)

Join our team as a Chronic Care Manager at Harris Computer. This role is part of an evergreen hiring process to build a talent pool for future opportunities. While there may not be an immediate vacancy, we encourage qualified candidates to apply for consideration.

Position Overview

The Nurse Chronic Care Coordinator will perform telephonic encounters with patients, develop detailed care plans within our electronic health record, and support proactive management of chronic conditions. This position is initially as an Independent 1099 Contractor with potential for full-time W2 employment with benefits.

Key Details
  • Location: Remote
  • Work hours: Flexible, minimum 20 hours/week, with a patient panel of at least 100 patients/month
  • Compensation: Productivity-based pay structure, starting at $10.00 per encounter, increasing with volume and consistency
Responsibilities
  • Follow care plans and practice orders
  • Provide prevention and intervention through motivational coaching
  • Develop positive patient interactions
  • Enhance revenue through billable CCM episodes
  • Create detailed care plans for prevention and intervention
  • Participate in scheduled meetings
Qualifications
  • Graduate from an accredited Nursing program (LPN, LVN, RN, BSN)
  • Current COMPACT license in the state of residence
  • At least 2 years of clinical experience in Med/Surg, Case Management, or home health
  • Experience with Electronic Medical Records and HIPAA compliance
  • Strong computer skills and high-speed internet
  • Excellent communication skills
