SOUTHERN HEALTH PARTNERS is launching a new correctional healthcare program in Cook County, Georgia, seeking a dependable Licensed Practical Nurse (LPN) to serve as Medical Team Administrator (MTA) at a new site. You’ll provide hands-on nursing care while coordinating daily healthcare needs in a growing program.

This role emphasizes autonomy, organization, teamwork, and delivering patient-focused care from day one.

Qualifications

• Current, unrestricted Georgia LPN license.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and organizational abilities.
• Ability to work independently and confidently make sound clinical decisions.
• Dependability and professionalism.
• Ability to collaborate effectively with a multidisciplinary team.
• Previous leadership or healthcare coordination experience is a plus.
• Correctional healthcare experience is welcome, but not required.

Responsibilities

• Provide direct nursing care to patients in a correctional healthcare setting.
• Complete nursing assessments and monitor patient conditions.
• Administer medications and treatments as ordered.
• Respond to urgent and emergent healthcare needs.
• Coordinate daily healthcare activities and help maintain continuity of care.
• Communicate with medical providers, correctional staff, and SHP leadership.
• Maintain accurate and timely clinical documentation.
• Support compliance with company policies, clinical standards, and regulatory requirements.
• Help establish organized and consistent healthcare processes at the new site.
• Promote a professional, respectful, and patient-focused healthcare environment.

As the LPN MTA, you’ll be an important part of establishing a consistent, organized, and patient-focused healthcare program from the start. You’ll provide hands-on nursing care while helping coordinate the day-to-day healthcare needs of the facility.
